What is a Care Plan?
Your Dovida Care Plan, sometimes referred to as a Support Plan, is a detailed document prepared by your Care Manager in conjunction with you, your family members, health care professionals, and anyone else who is part of the individual’s Circle of Care.
It describes the details of the care that is to be provided, and it is used by appropriately trained caregivers to deliver the right care, at the right time, based on their clients’ unique requirements.
Care Plans are regularly reviewed by Care Managers, and they can be adjusted as we get to know you, ensuring that you’ll always live Your Life, Your Way.

Care plan management means that you will have a dedicated Care Manager who will oversee your Care Plan, including your appointments, services, home modifications, equipment, and anything else that you may need for your care to run smoothly.
First, contact your local office, who will arrange a free, no-obligation care consultation.
A Care Plan will then be created, and your Care Manager will work closely with you to start organizing your care in a way that works for you. This will include creating a visit schedule.
Care Plan Management organizes the administration and scheduling of your care, advocates for you, and makes sure you stay informed. All of this is aimed at providing a smooth and efficient care service for you.
The cost of Care Plan Management is included in your service rates; there is no additional charge for it.
